APPLICATION PROCESS

The requirements of Part 1 - 3 must be met in order to accept your application.
Part 1.	Determine if your child is above the 80th percentile. Click here to calculate BMI using the CDC's online child & teen BMI Calculator. Part 2.	Determine if your child would benefit from the Camp
	w Child's age is between 10 - 16 w Child is willing to learn by being physically active in groups w Child is willing to try a structured nutrition plan

Part 3.	Complete a camp application form. Click here for 2009 Camp Divas Application. (Word document) Submit the completed form by mail, fax, or preferably by email for committee review.

Upon application acceptance, a CHOICES Staff person will contact you by phone.
