Ensure
that the child has a Physical before Camp starts |

Submit
a completed Physician Consent For to CHOICES |

Upon final acceptance, a
CHOICES Staff person will contact you by phone.
Additional details
and fees will be discussed at that
time.

Camp Fee is $100.00 per child enrolled. (cash, check or credit
card)
